Knowledge is power. The more you understand about the process, the more relaxed and prepared you will feel — and the better your result will be.

The most common thing I hear from clients who are interested in lip blush is: “I really want this but I am worried about cold sores.”

So let me reassure you straight away. Most people who follow the right preparation have absolutely no problem. And for those who do occasionally get a mild flare-up — it is manageable, it heals, and the result can still be corrected and perfected. This guide walks you through everything, step by step.

The cold sore virus — HSV-1 — is carried by around 70% of adults, often without knowing it. Here is the truth: lip blush will always trigger the cold sore virus if it is present in your system. The needle trauma near the lips is a direct stimulus — this is not a risk, it is a certainty. But — and this is the important part — with the right antiviral medication started before your appointment, a flare-up is preventable in the vast majority of cases. This guide tells you exactly what to do. You are in the right place.

Your Preparation Protocol

Think of this as your checklist. Follow it and you give yourself the best possible start — not just for cold sore prevention, but for the most beautiful healed result too. Everything here also helps your pigment last longer and your lips look their best on the day.

1
2 weeks before — start now
Hydrate your lips every single day
Apply a good lip treatment morning and night — and top it up through the day. Well-hydrated lips hold pigment better, heal faster, and are far more resilient. Think of it as building a strong skin barrier before we begin. The products below will help you choose what works for your budget.
2
2 weeks before — every morning
SPF lip balm every day — yes, even in winter
Sun exposure is one of the most common cold sore triggers. It also fades pigment. Getting into the habit of applying an SPF50 lip balm every morning protects both your colour and your lips long term. This one small habit makes a real difference.
3
2 weeks before — a few times a week
Gentle lip exfoliation
A gentle lip scrub 2 to 3 times a week removes dead skin so the pigment can sit evenly and smoothly. Nothing harsh — just a light, gentle scrub. Stop completely 48 hours before your appointment so the skin has time to settle nicely before we begin.
4
2 to 3 days before — if you have ever had a cold sore
Antiviral medication — the most important step
If you have ever had a cold sore — even just once, even years ago — please speak to your GP or pharmacist about antiviral medication before your appointment. This is the single thing that makes the biggest difference of all. Book your GP appointment as soon as you have your treatment date — do not leave it to the last minute. See the full section below on what to ask for.
5
24 hours before
No alcohol
Alcohol thins the blood, which means more bleeding during the procedure and can affect how the colour settles. One easy swap the evening before makes a genuine difference to your result.
6
Day of appointment
Arrive ready — moisturised, fed and feeling good
Clean, hydrated lips. No lipstick needed. Please eat before you come — never arrive fasted. If on that morning you notice any tingling or irritation on your lip, message me straight away. We reschedule without any fuss — no charge, no judgement, no stress. We simply wait until your lips are perfectly ready.

Antiviral Medication — What to Say to Your GP

This is the part that sounds complicated but is actually very simple. You do not need to know anything about dosage — your GP or pharmacist handles all of that. You just need to know what to ask.

Important — please read this carefully: Lip blush will always stimulate the cold sore virus. This is not a possibility — it is a certainty for anyone who carries HSV-1. The needle trauma near the lips is a direct trigger. This does not mean you cannot have lip blush — it means that antivirals are not optional if you have ever had a cold sore. They are essential. Even if you haven’t had an outbreak in years. Even if it was just once. Prevention is always easier than treatment.

Say exactly this to your GP or pharmacist: “I am having a cosmetic lip tattoo procedure and I have a history of cold sores. Can I have antiviral prophylaxis to prevent a flare-up?”

They will know exactly what to prescribe. Ideally start your antivirals 3 to 5 days before your appointment and continue for 5 days after. The two most common options are:

Valaciclovir antiviral lip blush cold sore prevention Bournemouth
Valaciclovir
Also known as Valtrex · Prescription only
The most commonly recommended option. Fewer tablets per day, highly effective. Requires a GP prescription — book in as soon as you have your appointment date.
Start 2 to 3 days before. Continue for 5 days after. Exact dosage from your GP.
Aciclovir tablets cold sore prevention lip blush
Aciclovir
Also known as Zovirax tablets · GP or pharmacist
A well-established option used for decades. Sometimes available from a pharmacist without a full GP appointment. Ask them directly.
Start 2 to 3 days before. Continue for 5 days after. Exact dosage from your GP or pharmacist.

Cannot get a GP appointment in time? No problem. You can get your antiviral prescription quickly and easily online — delivered to your door or collected from a Superdrug store.

A note from Skarlet

I know it can feel like extra effort to book a GP appointment just for this. But it genuinely makes the biggest difference between a worry-free healing journey and an unnecessary stress during the most important two weeks. Your lips deserve that investment — and so do you.

Cold Sore Stages — Spot It Early, Act Fast

The earlier you catch a cold sore, the easier it is to manage. The tingling you feel in Stage 1 — before anything is even visible — is your golden window. Acting there can stop the whole thing in its tracks. Here is what each stage looks like and what to do:

Cold sore stage 1 tingling prodrome
Stage 1
The Tingle
Tingling, itching or warmth. Nothing visible yet. This is your moment — act immediately.
Cold sore stage 2 redness
Stage 2
Redness
A pink or red patch appears. Still early. Apply antiviral cream now if you haven’t already.
Cold sore stage 3 bump
Stage 3
Small Bump
A small raised area forms. Tender to touch. Do not squeeze. Continue antivirals.
Cold sore stage 4 blisters
Stage 4
Blisters
Fluid-filled blisters appear. Most contagious stage. Keep clean, do not touch, continue antivirals.
Cold sore stage 5 open sore
Stage 5
Open Sore
Blisters burst and dry. Keep clean. Do not pick. Let it heal on its own.
Cold sore stage 6 scab
Stage 6
Scab Forms
A scab forms — healing underneath. Do not pick it off. Keep the area moisturised.
Cold sore stage 7 healed
Stage 7
All Clear
Skin heals completely. Redness fades. Virus goes dormant again. You are through it.

Feel the first tingle? Do this right now

Stage 1 — the tingle before anything appears — is your golden window. Acting here can stop the outbreak almost entirely. Do not wait to see if it gets worse.

  1. Apply Aciclovir cream (Zovirax) to the spot right now — 5 times today
  2. Call your GP or pharmacist for oral antivirals today — same day
  3. Message me straight away — I will guide you through every step
  4. Leave the area alone — no touching, no pressing, no picking

With antivirals started at Stage 1, the whole thing can resolve in 4 to 7 days instead of the typical 10 to 14. Earlier is always better. If a cold sore does affect the treated area during healing, it can cause a lighter patch — but this is correctable at your touch-up appointment, which is included in your treatment.

Lip Blush Healing — What to Expect Day by Day

This is one of the most important sections in this guide. Knowing what is completely normal during healing means you will not worry unnecessarily — and you will not be tempted to interfere at exactly the moments when leaving your lips alone matters most.

Lip blush healing day 1 immediate after
Day 0–1
Fresh and vivid
Colour is intense. Lips feel tender and slightly swollen. This is perfectly normal — your body is responding to the procedure.
Lip blush healing day 2-3 darkening
Days 2–3
Looks darker
The colour may appear even deeper than on day one. This is completely normal — it is the pigment oxidising as the skin begins to heal.
Lip blush healing days 4-7 peeling flaking
Days 4–7
Gentle peeling
The skin starts to shed very lightly — think of it as the lips renewing themselves. It is usually a mild flaking, not dramatic peeling. Do not pick it off.
Lip blush healing days 7-14 colour loss ghost phase
Days 7–14
Almost gone
The colour appears to almost disappear. Every single client goes through this — it is called the ghost phase. The pigment is anchoring into the deeper skin. It comes back.
Lip blush fully healed beautiful result
Days 14–21
Your real result
The true healed colour emerges. Soft, natural, exactly what we worked for. Touch-up appointment at week 4 to 6 to perfect anything that needs refining.

The healing summary — save this

1–3
Looks too dark — that is normal
The colour in the first few days is always more intense than the final result. Apply your healing balm every 2 to 3 hours. Keep your lips away from heat and sun.
4–7
Peeling phase — do not touch it
Light flaking happens as the skin renews itself. It is not dramatic — more like natural skin shedding. Let it happen on its own. Picking it off removes pigment with it.
7–14
Ghost phase — be patient
Colour looks like it has almost disappeared. This is the most common reason clients panic — and the most common reason I get worried messages. It is supposed to happen. The pigment is just settling deeper. Sit tight.
14+
The real result appears
Your true healed colour comes through — soft, even and natural. The touch-up at week 4 to 6 is where we assess the result together and refine anything. It is included in your treatment.

Simple Aftercare Rules

These rules exist to protect your colour and your healing. The first ten days are the most important — after that things become much more relaxed.

No kissing — friction disturbs the healing skin and can spread bacteria
No spicy, hot or acidic food — irritates the tissue and fades pigment faster. Keep food away from your lip line where possible
Drink through a straw — for the first few days this keeps liquid from sitting on the healing area. A small thing that helps a lot
No gym, saunas or steam rooms for the first week — heat and sweat open pores and draw pigment out
No sun on your lips during healing — UV fades fresh pigment very quickly
No lipstick or lip products other than your provided aftercare balm for the first 10 days
Do not pick the peeling — this is where most colour loss happens. Let it fall off naturally
Apply your aftercare balm every 2 to 3 hours — keeping the area moist is crucial for good colour retention
Continue your antiviral medication for the full prescribed course, even if you feel fine
Sleep on a clean pillowcase and try not to let your lips press against the pillow in the night

Cold sore and lip blush healing: If you experience a cold sore during healing, do not panic. Keep the area clean, apply Aciclovir cream immediately, contact your GP for oral antivirals, and message me. We simply wait until the skin is fully and completely healed before the touch-up — no redness, no tenderness, no trace. Then we correct anything that needs it.

Questions I Get Asked All the Time

I have never had a cold sore — do I still need to worry?
Around 70% of adults carry HSV-1 without ever knowing it. A first cold sore can occasionally be triggered by a lip procedure even with no previous history. It is worth mentioning to your pharmacist when you book — they can advise whether a precautionary antiviral is right for you. Better to ask the question than wonder later.
I feel a tingle but cannot get a GP appointment straight away — what do I do?
Go straight to a pharmacy. Pharmacists in the UK can provide Aciclovir cream (Zovirax) over the counter without a GP appointment — apply it immediately, 5 times today. Many pharmacies now also offer same-day consultations for antiviral tablet prescriptions. Boots Online Doctor and Superdrug Health Clinic are also quick options. Do not wait to see how it develops.
The colour has almost disappeared at week two — is something wrong?
Nothing is wrong — this is the ghost phase and every single client goes through it without exception. The surface layer of skin has shed and the pigment is anchoring into the deeper dermis. Your true colour will come through between weeks two and four. Please do not be alarmed. This is exactly why your touch-up appointment exists — we assess the fully healed result together and refine it from there.
Will my result still be ok if I had a cold sore during healing?
In most cases, yes — with a little patience. A cold sore during healing can sometimes cause a slightly lighter patch in the affected area, but this is correctable at the touch-up once the skin is completely healed. The key is to wait until there is absolutely no trace remaining before we touch it again. We never rush this.
I get cold sores quite frequently — is lip blush still possible for me?
Yes, and you are not alone in this. It is worth speaking to your GP about suppressive antiviral therapy — a low daily dose that significantly reduces how often outbreaks occur. Many people find this genuinely life-changing. Once frequency is better managed, lip blush with the right protocol in place works beautifully. We discuss your specific situation at consultation.
Beautiful lips are absolutely possible — even with a history of cold sores. Follow the protocol, trust the process, and let the result speak for itself.
