First of all — breathe. Having a history of cold sores does not mean you have to give up on the lips you have always wanted. It just means we do things properly from the start.
After ten years of doing lip blush in Bournemouth, I have guided hundreds of clients through this exact situation. The ones who follow the protocol? They heal beautifully, keep their colour longer, and rarely — if ever — have a flare-up. The ones who skip it? That is where the problems happen.
This guide covers everything. What causes a flare-up, what to do before and after, which products actually work, and exactly what to do in the unlikely event a cold sore appears. Simple, clear, and written so that anyone can follow it.
The most important thing to know: a cold sore appearing after lip blush is not a sign something went wrong. It is the skin reacting to the trauma of the procedure. It is preventable in most cases — and manageable in all of them. You are not alone in this, and you will be absolutely fine.
Why Does Lip Blush Trigger Cold Sores?
The cold sore virus — called HSV-1 (Herpes Simplex Virus Type 1) — is incredibly common. In fact, around 70% of adults carry it, and many people do not even know they have it. Once you have had it, it lives quietly in the nerve endings near your lips — not causing any problem most of the time.
Think of it like a sleeping cat. Most of the time it does nothing. But certain things wake it up:
Physical trauma to the lips — like the needle used during lip blush
Stress — physical or emotional
Sun exposure — UV light is one of the most common triggers
Being run down — illness, poor sleep, low immunity
Hormonal changes — including your menstrual cycle
Lip blush involves a fine needle depositing pigment into the lip tissue — which is naturally a stimulus for the virus. This does not mean you cannot have lip blush. It means we put the virus back to sleep before it has a chance to wake up. That is exactly what the protocol below does.

I always ask every lip blush client about their cold sore history at consultation — not to turn anyone away, but to make sure they are properly protected. Even if you have not had a cold sore in years, the virus can still be dormant in your system. The protocol I follow is the same whether you have had one outbreak or fifty. Prevention is always better than dealing with a flare-up during healing.

Before Your Appointment
Starting 7 to 10 days before — this is where great results begin
Everything you do before your appointment directly affects how your lips heal and how long your colour lasts. Think of this phase as building the foundation. Healthy, hydrated lips with a strong barrier hold pigment better, heal faster, and give the virus far less opportunity to reactivate.
Step 1 — Antivirals (if you have ever had a cold sore)
This is the single most important step. See the full antiviral section below.
Step 2 — Hydrate your lips intensively
Dry, cracked lips are more vulnerable to viral reactivation — because a damaged skin barrier gives the virus easier access to the surface. Start hydrating at least 2 weeks before your appointment, several times a day. See the product recommendations below.
Step 3 — Exfoliate gently
Removing dead skin helps the pigment sit evenly and gives you a smoother, more uniform result. Exfoliate gently twice a day — but stop completely 48 hours before your appointment so the skin has time to settle.
Step 4 — Protect from the sun
Sun exposure is a known trigger for cold sores — and sun damage also affects how pigment holds in the skin. Apply SPF lip balm every single day in the weeks before your appointment, even in winter. See recommendations below.
2 weeks before
Daily intensive lip hydration. SPF lip balm every day. Start antivirals if prescribed.
1 week before
Continue hydrating. No sun exposure on the lips. Gentle lip exfoliation daily.
48 hours before
Stop exfoliating. Keep lips moisturised. No alcohol.
Day of appointment
Arrive with clean, moisturised lips. Eat before you come. No lipstick needed.
If you have an active cold sore on the day — please reschedule. No exceptions. Working near an active outbreak can spread the virus and significantly damage your result. We wait a minimum of two weeks after the last scab has completely gone before rescheduling.
Antiviral Medication — The Most Important Step
If you have ever had a cold sore — even once, even ten years ago — antiviral medication before your lip blush appointment is not optional. It is the single most effective thing you can do to prevent a flare-up, and it works.
You need to speak to your GP or pharmacist and ask specifically for antiviral prophylaxis before a cosmetic lip procedure. They will know exactly what to give you. The two most commonly prescribed options are:
Valaciclovir
Also known as Valtrex · Prescription only
The most commonly recommended option for lip blush clients. Fewer tablets per day, better absorption. Ask your GP specifically for Valaciclovir for cold sore prevention before a cosmetic procedure.
Start 2 to 3 days before your appointment. Continue for 5 days after. Exact dosage from your GP or pharmacist.
Aciclovir
Also known as Zovirax tablets · GP or pharmacist
A well-established antiviral used for decades. Works by stopping the virus from replicating. Available from your GP or sometimes directly from a pharmacist.
Start 2 to 3 days before your appointment. Continue for 5 days after. Exact dosage from your GP or pharmacist.
Important: I cannot prescribe medication and neither can your lip blush artist. What I can do is tell you exactly what to ask your GP or pharmacist. Say this: “I am having a cosmetic lip tattoo procedure and I have a history of cold sores. Can I have antiviral prophylaxis to prevent a flare-up?” They will take it from there. Book your GP appointment as soon as you have your treatment date.

Understanding Cold Sore Stages — Know What You Are Looking At
The most important thing to know about cold sores is this: the earlier you act, the better the outcome. Knowing what each stage looks like means you can catch it at the very first sign — and stop it in its tracks before it becomes anything more than a tingle.
Here is what happens at each stage — and what to do. The stages your artist needs to know about most are Stage 1 and Stage 2 — before anything visible appears. That tingling feeling is your signal to act immediately.
Stage 1
The Tingle
Tingling, itching or burning sensation. Nothing visible yet. This is your golden window — act NOW.
Stage 2
Redness
A red patch appears. Slight swelling begins. Still early — antiviral cream immediately.
Stage 3
Small Bump
A small raised bump forms. Virus is replicating. Painful to touch. Do not squeeze.
Stage 4
Blisters
Fluid-filled blisters form. Most contagious stage. Do not touch. Continue antivirals.
Stage 5
Open Sore
Blisters burst. Shallow, painful open area. Keep clean. Do not pick. Let it dry naturally.
Stage 6
Scab Forms
A yellow-brown scab forms. Healing is happening underneath. Do NOT pick it off.
Stage 7
Healing
Skin regenerates. Redness fades. Virus goes dormant again. You are through it.
The full cycle from first tingle to fully healed is typically 7 to 14 days. With antivirals started at Stage 1, this can be shortened to 4 to 7 days and the severity is significantly reduced. This is why catching it early is everything.
What does this mean for your lip blush result? If a cold sore appears in the treated area during healing, it can affect pigment retention in that spot — leaving a lighter patch. This sounds worse than it is. It can always be corrected at your touch-up appointment once the skin has completely healed. Your result is not ruined — it just needs a little extra attention at the follow-up.

The Day of Your Appointment
What to expect and how to arrive
Your lips will be numb before we begin — so discomfort is minimal. The procedure takes between 1.5 and 2.5 hours including design, colour matching and numbing time. Arrive with moisturised, clean lips. Eat before you come. No lipstick needed.

After Your Treatment
Days 1 to 10 — the healing window that determines your result
What you do in the first ten days after your lip blush has a direct impact on how the colour heals, how long it lasts, and whether a cold sore has any opportunity to appear. This phase is where your investment is either protected or compromised.
Days 1–3
Bold and vivid
Colour looks intense. Some swelling and tenderness. Completely normal. Apply healing balm every 2 to 3 hours.
Days 2–4
Tight & dry
Lips feel tight and dry. Apply balm on completely dry lips only. Keep washing morning and night.
Days 4–7
Peeling begins
Skin flakes. Colour underneath looks light and patchy. Do NOT pick or peel — this is critical.
Week 2–3
The ghost phase
Colour almost disappears. Every single client goes through this. The pigment is anchoring deeper. It will come back.
Week 4–6
True result 🌸
Your real colour emerges. Soft, beautiful, natural. Touch-up at 6–8 weeks perfects the final result.
The rules for these 10 days
No kissing — friction and bacteria are not your friend right now
No spicy, hot or acidic food — irritates the healing tissue and fades pigment
No sun exposure on the lips — UV fades pigment rapidly during healing
No steam rooms, saunas or intense exercise — heat opens pores and draws pigment out
No lipstick, lip liner or any lip products other than your aftercare balm
Do not pick the peeling skin — this is where most colour loss happens
Continue your antiviral medication for the full prescribed course
Apply healing balm every 2 to 3 hours — Aquaphor or Dr Pawpaw
Drink through a straw where possible for the first few days
Eat with a fork and keep food away from your lip line
If a Cold Sore Appears — What to Do Right Now
You feel the first tingle — act in the next 60 minutes
The prodrome stage (Stage 1 — the tingle before anything visible appears) is your golden window. Antiviral medication taken at the very first sign can stop the outbreak entirely or reduce it to almost nothing. Do not wait to see if it develops. Act immediately.
- Apply Aciclovir cream (Zovirax) to the area immediately — 5 times today
- Contact your GP or pharmacist for oral antiviral medication today — same day
- Message me straight away — I will guide you through everything
- Do not touch, press or interfere with the area
- Keep lips clean and gently moisturised
If a cold sore does appear despite the protocol — and it occasionally still does even with everything done correctly — here is what happens next:
Start oral antivirals immediately — Valaciclovir or Aciclovir from your GP
Apply Aciclovir cream (Zovirax) directly to the area 5 times a day
Leave it completely alone — no picking, no pressing, no drying agents
Keep the area gently clean with water only
Message me — I will monitor your healing with you and we adjust your touch-up date
Do not panic — it heals, it resolves, and the result can still be corrected at the touch-up
Your touch-up appointment at 4 to 6 weeks is included and is exactly what it is for. If a cold sore affected any area of pigment retention, we correct it there. No extra charge. No drama. We just wait until the skin is completely, 100% healed before touching it again.

Long-Term Maintenance
Protecting your colour and preventing future outbreaks — for good
The habits you build after your lip blush directly affect how long your colour lasts and how rarely cold sores recur. Three simple daily habits make the biggest difference of all.
Daily SPF
Every single day, forever. Uvistat SPF50 or Nivea SPF50. UV is the number one cause of both pigment fading and cold sore recurrence. This is the most impactful habit you can build.
Daily hydration
A hydrated lip barrier is a protected lip barrier. Your Laneige or Dr Pawpaw overnight — every night. Takes 10 seconds. Makes a significant difference.
Act on the tingle
The moment you feel any tingling — apply Aciclovir cream immediately. Do not wait. Early action during the prodrome stage is the difference between a minor inconvenience and a full outbreak.
Frequent outbreaks
If you get cold sores regularly (more than 4 times a year), speak to your GP about suppressive antiviral therapy — a low daily dose that significantly reduces recurrence. Life-changing for some clients.
Your Questions Answered
I have never had a cold sore. Do I still need to worry?
You could still carry HSV-1 without ever having had a visible outbreak — around 70% of adults carry the virus, many without knowing. Your first ever flare-up could potentially be triggered by the trauma of lip blush. This is why I include cold sore protocol information for all lip blush clients, not just those with a history. A conversation with your pharmacist about antiviral precautions is always worth having before any lip blush appointment.
What if I feel the tingle but I cannot get a GP appointment immediately?
Go directly to a pharmacy. Pharmacists in the UK can now prescribe Aciclovir cream (Zovirax) over the counter without a GP appointment. Apply it immediately — 5 times a day. If you need oral antivirals and cannot reach your GP, many pharmacies now offer same-day consultations for antiviral prescriptions, or you can use an online GP service like Boots Online Doctor, Superdrug Health Clinic or Push Doctor. Do not wait.
I had a cold sore. Will my lip blush result still be ok?
In most cases, yes — with some patience. A cold sore during healing can cause lighter patches in the affected area, but this is correctable at your touch-up appointment once the skin has completely healed. The touch-up is included and is exactly where we address anything like this. The key is to let the cold sore heal completely — not a trace of scab remaining — before we assess or touch anything. Skin that is not 100% healed cannot be worked on safely.
How long after a cold sore can I have my touch-up?
A minimum of two weeks after the last scab has completely gone and the skin feels and looks fully normal. There should be no redness, no tenderness, no visible sign that anything was there. I assess this at the touch-up appointment — if I am not satisfied the skin is ready, we give it more time. A result done too soon over incompletely healed skin will not hold properly and could cause further irritation.
Will my colour be uneven if I had a cold sore?
It may be slightly lighter in the affected area — because the cold sore disrupts the skin during the critical healing window when pigment is anchoring into the dermis. This is not permanent and it is correctable. At your touch-up we assess the healed result across the whole lip and add pigment where needed. This is precisely why the touch-up appointment exists — skin does not always heal uniformly, cold sore or not.
I am nervous. Should I still go ahead with lip blush?
The nervousness is completely understandable — and completely normal. But the protocol works. The vast majority of clients who follow it go through their lip blush healing with no cold sore whatsoever. The ones who do occasionally get one manage it, get through it, and end up with beautiful lips on the other side. Come in for a consultation.
